some can, some can't or will be an extremely expensive operation. If the crank was originally ordered as an internally balanced deal this usually includes includes longer rods and lighter weight modern pistons. The longer rods allow for larger counterweights and the lighter pistons reduce the amount of counterweight needed to begin with.

If you have an externally balanced setup it can be converted to internal balance, but the shop has to drill out the counterweights and install slugs of heavy metal, AKA Mallory metal. this allows use of non-counterweighted balancer and flywheel.

If you are running a street type engine the benefits of internal balance are largely theoretical, don't lose any sleep over it.
