Does anyone know of a repair plate for a 1969 radio opening that was widened ??

I've never seen one. You could buy a junk dash with a good radio opening from someone and plastic weld in the area you need. Take a look at this thread for as dash with a good radio opening: http://www.chevelles.com/forums/showthread.php?t=352725
