I have the factory wiring harness and what I believe is the factory 3-pronged switch. Does anyone know where it mounts and what the bezel/mounting bracket looks like? Does anyone in the aftermarket make a bezel? Thanks!

The switch is the same as the rear window switch on a wagon. It mounts on the lower left side of the dash. I think they all have the holes for the switch don't they? The bezel is a trapazoidish chrome piece with a rectangular hole for the switch and two holes to screw it into the dash.
I bought a '67 impala switch when mine went bad. It's actually nicer cause the button is chrome. Looks nice. They are tough to find.

Look at this pic of my interior and if you start at the brake pedal and follow the arm up to the dash... the chrome edge you see is the bezel of the top switch.
