I have been driving myself nuts trying to decode this. Some of it I have been able to figure out.

What is the trim code 753? And the last line after the 04c?

after 04C are plant codes that no one has been able to decode and were for internal tracking by the production plant, in your case Baltimore.
The 04C stands for:
Your car was built in the fourth month of 1969 in the third week of the month. If you go into goggle and type in "1969 calendar" you find that the third week of April was April 13th thru April 19th. Your car was therefore build in April 13-19, 1969 Baltimore

Production weeks do not always fit with calendar weeks.
