Did these hoods come on 70-72 SS cars or just the Heavy Chevys(no flapper)? I took one off a 72 parts car I had, thought it was a Heavy but crushed it before I figured it out (real real bad shape.)

I have a 71 Malibu, originally with a 307,350 trans and a 12 bolt open. Is the 12 bolt rare for this type of car?

What you have is that standard SS option package "domed hood". Cowl Induction was an extra cost option on top of the SS package as was stipes on the std SS hood (stripes came as part of the CI option but could be deleted).

12 bolts seem common non SS cars though generally with optional engines. I have a relative that bought a new '70 base model 4-door with the 6 cylinder and it had an open 12-bolt! But this is probably not very common. Generally any car with a 350 4-bbl carb or larger engine came with them. Posi was a separate option. Quite common in elcaminos and monte carlos. At least from my direct experience and observations.
