My driver side door is getting hard to close properly. Don't think the bushings have every been replaced. Door is starting to sag a little.
Any advice on replacing them?
Does the fender need to come off or can I replace it with the door on?

You can replace them without taking the fender off.
Depending if the holes on the bracket are wallowed out you might need to mill the holes out (to make them round again) and put in some bushing.
Go to your hardware store or auto parts store, after you find the bushing then you will know what to machine the holes out too.
I've done this myself, using a BridgePort mill and a long releived end mill that I ground to the size I needed for the bushings I was using.

OK.. Pardon my ignorance here.
What is a Bridgeport mill?
What kind of bushing is this? I was not aware of there being different sizes. I thought the only ones availiable was the stock ones.
Can you explain a little..more?

A BridgePort is a manual mill, like you would find in a machine shop.
Yes there are standard sizes, but depending on how bad the holes are wallowed out you may need to go to a larger bushing. Just make sure the pin fits the I.D. of the bushing.
If your doors already have bushing in them, then you can just install new bushing.
If your doors don't have bushing and the holes are wallowed then it will need the holes machined out for bushing.
If you have to machine the brackets for new bushing, it's best to have it done on a mill using an end mill. Tring to drill the holes will not work because the drill will walk off causing the holes not to be aligned and the hole will be oversized for the bushing.
